This waiver is meant to confirm your awareness and acceptance of the following terms in working with The Baking Dietitian, LLC (aka Dietitian Andie).
i. I understand and agree that although all of The Baking Dietitian, LLC programs are reviewed by a Registered Dietitian, I have checked with my health care team to make sure that I am healthy enough to begin a virtual nutrition program.
ii. I understand that the services, communications, materials, and information provided by The Baking Dietitian, LLC are in no way intended to diagnose, cure, or treat any medical or health condition. Our services will not be used in place of my physician’s recommendations.
iii. I completely understand that The Baking Dietitian, LLC services and materials do not constitute or create a doctor-patient, therapist-patient, or other healthcare professional relationship.
iv. I understand that The Baking Dietitian, LLC is not responsible for any injury occurred in relation to the programs I am involved in.
v. I understand The Baking Dietitian, LLC owns the rights to the materials presented to me and I will not distribute or recreate materials or information to others under any other name.
vi. I understand that all program purchases with The Baking Dietitian, LLC are non- refundable. Fees for this program are due in full or in part according to our payment plan. Payment can be made via credit card or PayPal account.
vii. I am 100% committed and ready to take the steps to prioritize this program to achieve my goals.
By signing below, I agree that I have read and understand the above information, and agree to the terms of service stated above. I understand I have the right to stop following the program at any time. I understand that it is usually best for Service Providers and clients to make joint decisions about termination. My signature indicates that I am giving my consent for support in the Healthy Habits Nutrition Coaching Program.
